One size is required. The rest are optional, and mostly exist for hardware people no longer carry.

1320 × 2868 portrait, the 6.9" size. Supply that and you have satisfied Apple for every iPhone.

Why two numbers appear in some rows
Apple accepts more than one resolution per display class because devices in that class shipped with different panels. The 6.1" row spans three generations of hardware with genuinely different pixel counts. Any listed value is valid — pick one and stay consistent across the set, because mixed dimensions inside a single display size are rejected.
The 4" and 3.5" trap
Those rows list two variants in Apple's own spec: one with the status bar and one without (640 × 1136 versus 640 × 1096). If you are supporting hardware that old you are almost certainly maintaining a legacy build, and the without-status-bar variant is the safer upload.
You do not need a screenshot per device. Apple downscales your 6.9" set for smaller screens. Uploading every optional size multiplies your localization work by eight and changes almost nothing about how the listing looks.
